Fresh (First Application)

• Minor (under 18 years):

1. Completed online application form (names and date of birth on the application FORM MUST BE IN THE SAME ORDER ON BIRTH CERTIFICATE AND NIN)
2. Passport Payment slip
3. One (1) Passport Photograph
4. Clean copy of Birth Certificate (Original may be required for sighting)
5. Clean copy of parent’s passport data page
6. Consent letter signed by parent and duly notarized by notary officer
7. Self-addressed return envelope with appropriate stamp from Post Office
8. National Identification Number (NIN)
9. Appointment slip

• Adult (18 years and above):

1. Completed online application form (names and date of birth on the application form MUST BE IN THE SAME ORDER ON BIRTH CERTIFICATE AND NIN)
2. Passport Payment slip
3. Evidence of Nigerian citizenship (state or local government identification certificate or evidence of birth by parent with Nigerian citizenship by birth)
4. One (1) Passport Photograph
5. Clean copy of Birth Certificate (Original required for sighting)
6. Clean copy of parents’ Nigerian passport data page in lieu of No. 3 above
7. Self-addressed return envelope with appropriate stamp from Post Office
8. National Identification Number (NIN)
9. Appointment slip

Renewal

1. Completed online application form (names and date of birth on the application form MUST BE IN THE SAME ORDER on current Passport and NIN)
2. Passport payment slip
3. Copy of current (expired/expiring) passport data page
4. Expired/expiring passport booklet (if not presented will be treated as Lost)
5. Self-addressed return envelope with appropriate stamp from Post Office
6. National Identification Number (NIN)
7. Appointment slip

Correction of Name/Data on Passport

• Change of Name due to Marriage/Divorce:

1. Completed online application form (names and date of birth on the application form MUST BE IN THE NEW NAME AND SAME on the NIN)
2. Passport Payment slip
3. Copy of current (expired/expiring) Passport data page
4. One (1) Passport Photograph
5. Expired/expiring Passport booklet
6. Clean copy of marriage certificate or divorce documents as applicable
7. Copy of Husband’s means of Identity
8. Self-addressed return envelope with appropriate stamp from Post Office
9. National Identification Number (NIN)
10. Appointment slip

• Change of Name/Correction of Error on request:

1. Completed online application form (names and date of birth on the application form MUST BE IN THE NEW DATA AND SAME on the NIN)
2. A formal application letter requesting for correction to be made and stating reason for change
3. Passport Payment slip
4. One (1) Passport Photograph
5. Copy of current (expired/expiring) Passport data page
6. Expired/expiring Passport booklet
7. Court order for change of names and newspaper publication
8. Copies of previous documentary evidence to support request

Replacement of Lost or Stolen Passport

1. Completed online application form (names and date of birth on the application form MUST BE IN THE SAME ORDER on MISSING Passport and NIN)
2. Passport payment slip
3. One (1) Passport Photograph
4. Clean copy of Police Report
5. Clean copy of lost/stolen passport data page (Lost/Stolen Passport number is a must, if data page is not available)
6. Self-addressed return envelope with appropriate stamp from Post Office
7. National Identification Number (NIN)

Replacement of Damaged Passport

1. Completed online application form (names and date of birth on the application form MUST BE IN THE SAME ORDER on MISSING Passport and NIN)
2. Passport payment slip
3. One (1) Passport Photograph
4. Copy of damaged passport data page
5. Remnant of Damaged passport booklet (if not presented will be treated as Lost)
6. Fire incident report if damaged by fire
7. Written application letter explaining reason for damage of the booklet.
